LIFE-NWPA is centered around a successful model the “It is better for the health of seniors with chronic care needs to be served in the community, if possible.”
The LIFE-NWPA Program is nationally known as the Program of all Inclusive Care for the Elderly or PACE. Since Pennsylvania has a “PACE” drug program already in, Pennsylvania calls the Program of all Inclusive Care for the Elderly, LIFE or Living Independence for the Elderly.
As we age, seniors may develop chronic care needs and some of these needs may only be addressed by the move to a nursing home. The state of PA wants to give seniors the choice to stay in their own homes; therefore the LIFE of NWPA may be able to help you. The program is an alternative to a nursing home.
The LIFE Program provides community based care and services to people who otherwise need nursing home level of care. The LIFE Program provides participants, their families, caregivers and professional health care provider’s flexibility to meet the participant’s needs and to help them to continue to live safely in the community and keep them in their own residence.
Who is eligible to be a LIFE Participant?
- Must be 55 years old or older.
- Live in Erie County.
- Certified by the State as Nursing Home Clinically Eligible.
- Financially eligible for Medical Assistance or able to pay privately.
- Able to live safely in the community when you join with the help of LIFE Program Services.
